In this essential position, you will work with the Director of Nursing, clinical staff, and front office team to ensure coordinated patient care and support to our healthcare professionals. Job Summary Hospice LVN provides nursing care to terminally ill Hospice patients as needed. You will evaluate the patient’s overall health and aim to improve quality of life. All nursing will be done in accordance with each patient’s individual plan of care. Responsibilities Provide services within the scope of Nursing, in accordance with the plan of care, and in coordination with the interdisciplinary team Follow plan of care; provide treatment; evaluate patient’s reaction to treatment Report changes in the patient’s condition to the Hospice Clinical Director Online charting in Kinnser on company-provided devices Follow all company policies and procedures Perform other duties as assigned Requirements One year of nursing experience One year of Hospice or Home Health experience (preferred but not required) Current LVN License to practice in California Current CA Driver’s License #J-18808-Ljbffr Health Link Home Health and Hospice Agency
